Client Care Coordinator
Responsabilities
- Promote the Vision, Mission, Values and Goals of the company.
- Conduct and participate in marketing/outreach efforts as required.
- Follow up on leads and referrals from family members, caregivers, and other referral sources.
- Build and maintain relationships with contract referral sources. Generate home care patient referrals from existing and potential accounts.
- Document all notes and tasks in the system accurately and timely.
- Liaison between the agency, insurance plan, and patients.
- Follow guidelines and compliance procedures implemented by Ideal Home Health.
- Proactively resolve, manage, communicate, and address matters related to patients and/or caregivers.
- Manage a high volume of incoming and outgoing calls.
