Client Care Coordinator

Responsabilities

  • Promote the Vision, Mission, Values and Goals of the company.
  • Conduct and participate in marketing/outreach efforts as required.
  • Follow up on leads and referrals from family members, caregivers, and other referral sources.
  • Build and maintain relationships with contract referral sources. Generate home care patient referrals from existing and potential accounts.
  • Document all notes and tasks in the system accurately and timely.
  • Liaison between the agency, insurance plan, and patients.
  • Follow guidelines and compliance procedures implemented by Ideal Home Health.
  • Proactively resolve, manage, communicate, and address matters related to patients and/or caregivers.
  • Manage a high volume of incoming and outgoing calls.
