To be eligible for CDPAP, consumers must have the necessary requirements:

- Have Medicaid
- Need home care
- Be a self-directing person, or have someone who can direct care for them
Usually, to enroll in this program, consumers should be enrolled in Medicaid Managed Care. On your part, you may have the benefit of choosing which type of plan you would like to enroll in.

There are several steps involved in the process and it takes more than filling out just one application form. There are three steps involved:
- Assessment – where a nurse assesses applicants before they can be enrolled in the program
- Patient Forms – the patients are required to fill these out.
- Personal Assistant Forms – these are the forms the personal assistant will need to fill out.

Every CDPAP consumer is different. The amount of home care they get varies. The amount of hours you will receive is determined by your Medicaid Managed Care plan.
CDPAP workers are compensated. How you will be compensated depends on many different factors. If you want to have more information, call 646-747-0181.
Consumers of CDPAP can hire nearly anyone that is allowed to assist them as their caregiver. This includes majority of their family members and friends. There are certain exceptions, however, such as spouses being unable to provide care for each other and parents being ineligible to provide care for their children under 21 years of age.
CDPAP caregivers do not need to be certified or have licenses. They are trained by the consumers themselves or the consumers’ representatives.
Compared to Home Health Aides and Personal Care Aides who are only allowed to provide personal care services such as bathing, assistance with dressing, personal hygiene, shopping, paying bills. The CDPAP Personal Assistants can provide assistance with skilled care such as wound care, trach care, and giving insulin shots.
You may be asking the question, “What happens if I don’t have Medicaid?” Well, since CDPAP is a Medicaid-based program, you can only enroll if you have Medicaid. There is no need to worry if you do not have it because we can be there to help you with the process of applying for Medicaid.
